I started my Hepatitis B series and sent in the Statement to Admissions on time. Now, how do I prove that I have finished the series and avoid any delay in my registration?
You will need to complete another UCSD Hepatitis B Statement online via TritonLink/MyApplication by March 2009.
How long does it take to complete the series of shots?
Full immunization for Hepatitis B requires a series of 3 shots administered over a period of approximately 4-6 months. The 2nd shot is due 1-2 months after the 1st and the 3rd shot is due 4-6 months after the 1st.

I got my 2nd shot more than 2 months after the first. How long do I have to wait to get the 3rd shot?
There must be at least 2 months between the 2nd and 3rd shots., and the 3rd shot cannot be given sooner than 4 months after the 1st.
May I document my immunization by providing a note or other form from my healthcare provider?
No, you must complete the form.
Can I get the shots at UCSD Student Health Service?
Yes, in-coming students may receive the immunizations at UCSD SHS. You must provide a letter of acceptance and proof of intent to register. All services provided prior to Fall Quarter are on a fee-for-service basis including Hepatitis B immunization. If you are under 18 years of age, a parent or guardian must sign a Consent to Treat form before SHS may provide any services (form available on SHS website http://studenthealth.ucsd.edu).
What is the cost of Hepatitis B immunizations?
The cost may vary. Currently SHS charges $53 ($35 per shot plus $18 nursing visit charge) Note: nursing visit charge will increase to $25 in Fall 2008) There is no visit charge during the Academic year in which you are a registered student. (The vaccine is $25 if covered under SHIP; deductible is waived.) Prices are subject to change.
What if I cannot afford to pay for the immunizations?
Public Health may provide the vaccine. Also, if you are unable to acquire the shots before September, UCSD SHS accepts VISA, MasterCard and Discover credit cards. In hardship cases, the student's UCSD account may be billed along with an administrative fee.
I don't have records of when I received the vaccine doses. What can I do?
If you are unable to obtain your records, you will need to contact a physician who can perform a test to determine if you are already immune or still susceptible to the Hepatitis B virus. This test can also be performed at SHS (if you are a registered student or for incoming students with a letter of acceptance and proof of intent to register). The current fee for the blood test is $4.75 (subject to change) plus $28 nursing visit and lab fees.
When does Admissions need to receive the completed form to avoid a hold on my registration?
Admissions must receive the form, on-line, by July 15, 2008. This is the final deadline.
What will happen if I do not complete the form and at least start the series of shots by Fall Quarter?
A hold will be placed on the student's record blocking the cashier from processing any payments to your account. Payments will be returned to sender. A late registration fee of $50 may be assessed. If you do not respond by the end of the second week of instruction, you will have all enrollments cancelled and will not be a student at UCSD for the term.
