Medical Records

Requests may be done in person, by mail, or by fax. Email is not accepted. You may request in person, Monday through Friday from 9:00 am to 12:00 pm and 1:00 pm to 4:00 pm. If you need to communicate by mail or fax, please download and complete the Authorization for Release of Health Information Form and fax to (858) 534-7545.

Medical Records sent to SHS from an outside provider or facility should be mailed to the address above.

Time and Costs

Immunization records may be released at the time of request. For all other requests, it may take up to 10 business days to complete.

Fees for medical records are as follows:

Records Onsite:
The first 4 pages are free, thereafter .15 per page, plus a $5.00 administrative fee.

Records Offsite:

The first 4 pages are free, thereafter .15 per page, plus a $7.50 administrative fee.

There is no charge when SHS forwards medical record copies directly to another health facility or clinic.

Immunization Records

We only maintain records of immunizations that you acquire at SHS or records sent to us to file in your medical chart.

Lab Results

Each group within SHS maintains copies of lab results for 3 weeks only. Thereafter, you will need to follow the standard procedure for requesting medical records by completing an Authorization for Release of Health Information Form and following the instructions above.
