Medical Records

Requests may be done in person, by mail, or by fax. E-mail requests are not accepted. You may request in person, Monday through Friday from 9:00 am to 12:00 pm and 1:00 pm to 4:00 pm.

If you need to communicate by mail or fax, please download and complete the Authorization for Release of Health Information Form and fax to (858) 534-7545.

Medical records sent to SHS from an outside provider or facility should be mailed to the above address.

Time and Costs

Immunization records may be released at the time of request. For all other requests, it may take up to 7 business days to complete.

Fees for medical records are as follows:

Records On-site:
The first 4 pages are free, thereafter .15 per page, plus a $5.00 administrative fee.

Records Off-site:

The first 4 pages are free, thereafter .15 per page, plus a $7.50 administrative fee.

There is no charge when SHS forwards medical record copies directly to another health facility or clinic.

Immunization Records

We only maintain records of immunizations that you acquire at SHS or records sent to us to file in your medical chart.

Lab Results

Lab results are available for pick up in the Medical Records Department only after review and approval by one of our providers. There is no charge for this service, and a written authorization for release is not required. However, a valid picture ID must be presented at time of request.

If you need an explanation of the lab results, you should contact the provider who ordered the tests.
