Patient Rights and Responsibilities
As a patient of the UCSD Student Health Services
YOU HAVE THE RIGHT TO:
- Receive considerate and respectful care
- Receive complete, current, and clear information about your diagnosis, treatment, and prognosis
- Give informed consent prior to the start of any procedure and/or treatment
- Change primary or specialty health care provider, if available
- Refuse treatment to the extent permitted by law
- Confidentiality as prescribed by law
- Consent or refuse to participate in research projects affecting your care
- Know about any fees for service
- Know the appropriate channels for expressing comments and grievances about services
YOU HAVE THE RESPONSIBLITY TO:
- Show respect for staff and other patients
- Provide information needed to assure proper evaluation and care
- Ask questions about your care
- Understand the prescribed treatment
- Give feedback about policies and quality of care
- Take an active role in your health through health education materials and programs
- Participate fully in your health care
