As a patient of the UCSD Student Health Service

YOU HAVE THE RIGHT TO:

  • Receive considerate and respectful care
  • Receive complete, current, and clear information about your diagnosis, treatment, and prognosis
  • Give informed consent prior to the start of any procedure and/or treatment
  • Change primary or specialty health care provider, if available
  • Refuse treatment to the extent permitted by law
  • Confidentiality as prescribed by law
  • Consent or refuse to participate in research projects affecting your care
  • Know about any fees for service
  • Know the appropriate channels for expressing comments and grievances about services

YOU HAVE THE RESPONSIBLITY TO:

  • Show respect for staff and other patients
  • Provide information needed to assure proper evaluation and care
  • Ask questions about your care
  • Understand the prescribed treatment
  • Give feedback about policies and quality of care
  • Take an active role in your health through health education materials and programs
  • Participate fully in your health care
