SHIP Claim Information
Although the Student Health Insurance Office can provide assistance with filing and understanding claims, we do not process the claims. The Student Health Insurance Plan (SHIP) uses Personal Insurance Administrators, Inc. (PIA) as a third party claims administrator to process all SHIP and DHIP claims.

All claims should be sent to PIA directly at the address above. Claims should include the following information: an itemized bill(s) (including diagnosis) and completed claim form. Be sure to include your full name, address, UCSD Personal Identification Number (PID), date of birth, and your undergraduate or graduate policy number on the claim form.

How can I get a claim form?
Claims forms are available at the Student Health Insurance office or you may print a claim form by clicking on the link below. This form will open up in a new window as a .pdf document that you can print for mailing. Attach all receipts or bills, make copies for yourself and mail to the address listed on the top left of the claim form.
What do I do if I receive a bill?
You may contact Personal Insurance Administrators, Inc. (PIA) at 1-800-468-4343, to see if a claim has already been submitted or processed. If they have not received a claim for the bills in question, you will need to file the claim yourself or provide your insurance information to the provider if they agree to file the claim on your behalf.
