The Dependent Health Insurance Plan (DHIP) is a voluntary plan which provides medical and dental benefits for eligible dependents of students enrolled in SHIP.
An eligible dependent is the insured student's 1) legally married spouse who resides with the student or an insured student's Domestic Partner; or 2) child who is dependent upon the student for support and maintenance and is under the age of 19 (23 if a full-time student).
Coverage for eligible dependents will not be effective prior to that of the insured student, or extend beyond that of the insured student. Enrollment/re-enrollment in DHIP is not automatic. Eligible dependents must be re-enrolled each subsequent quarter in order to continue coverage under DHIP.

Enrollment forms will not be accepted after the enrollment deadline.

Costs of 2007-2008 DHIP
Cost of Undergraduate Dependent Coverage per quarter:
- Spouse or Domestic Partner $777.00
- Child(ren) $520.00
Cost of Graduate Dependent Coverage per quarter:
- Spouse or Domestic Partner $943.00
- Child(ren) $458.00
Plan Changes for 2007-2008 DHIP
- Incorporated pre-certification and continued stay review for inpatient stays, and
- Incorporated pre-certification and continued stay review for inpatient stays, and pre-certification for surgery. Penalties apply for non-compliance. (see Page 9 of the SHIP brochure for complete details.)
- Separated deductible: $300 for outpatient and $300 for inpatient.
- Office visit copay is $15.00.
- Emergency room copay is $50.00.
- Prescription copay for non-SHS, non-formulary drugs is 50%.
- Lifetime maximum: $30,000 per illness or injury.
- Dental: 1st prophylaxis at 100%, deductible waived. 2nd prophylaxis remains at 80%, after deductible.
