Health Requirements for Students

The health of the individual can affect the health of the campus community, and UC San Diego is committed to protecting the well-being of all our students. All incoming and re-admitted graduate and undergraduate students are required to meet the UC Immunization and Tuberculosis (TB) Screening Requirements.

”Incoming” students includes: 
  • New Undergraduate Students 
  • New Graduate Students 
  • UCSD Undergraduates Transferring to Graduate Study 
  • Transfer Students 
  • Re-admitting Students 

To complete your Health Requirements, you will first need to activate your online health portal (using your Student AD) in MyStudentChart. Please see the MyStudentChart Instructions (PDF) for further explanation on the setup process. You can communicate with the Nursing team by going to Ask-A-Nurse in messages.

Please complete these requirements early to avoid delays. Clearance can take 7-10 days after receipt. FAILURE TO COMPLETE REQUIREMENTS WILL IMPACT YOUR ABILITY TO ENROLL IN CLASSES.

Tuberculosis Requirement: TB Risk Screening Questionnaire

  • All incoming students are required to complete a TB Risk Screening Questionnaire first.
  • For those at higher risk, TB testing will also be required. Risk is determined by the questionnaire. Take the TB Testing form to your Healthcare provider if you are directed to do so after completing the questionnaire.

  • All incoming students are required to submit vaccine/immunization dates and upload a vaccine record, as per University of California policy. 
  • Please obtain records OR have your healthcare provider fill out the UCSD Immunization Form. 

COVID-19 Vaccination

  • Since the University’s COVID-19 vaccine policy was first issued in December 2020, UC has continued to review and adjust UC’s COVID-19 vaccination requirements as public health conditions have evolved. With the changes to federal and state public health guidance and the end of the federal Public Health Emergency on May 11, 2023, UC has moved to a systemwide COVID-19 vaccination opt-out program for UC students. 
  • Under the revised policy, the University requires students to either receive or affirmatively decline the most updated COVID-19 vaccination annually in the Fall/Winter.

Influenza Vaccination

  • All students, regardless of age are to receive or affirmatively decline the influenza vaccine annually each Fall/Winter.

Exemption Information

Medical Exemption

A Medical Exemption can be requested to allow an exemption consistent with Centers for Disease Control and Prevention (CDC) guidance and manufacturer labeling on contraindications and precautions. 

Students with a medical condition having a contraindication to the MMR, Varicella, Tdap, Meningococcal, or COVID-19 vaccines will need their provider to complete the Student Immunization Exemption Request Form. Your form needs to be signed by an MD, DO, PA or NP please.

Religious Exemption

As of January 1, 2025, the University of California will permit students to request exception to vaccination based on religious belief.

Extension (International Program)

Deadlines will be handled by your program dependent upon program start dates. 

Please email the Extension Program with any issues, DESHealthandSafety@ucsd.edu.

Tuberculosis Requirement:

  1. Sign up for MyStudentChart, if needed follow instructions.
  2. On the MyStudentChart Welcome Page go to Menu > Immunizations and Screening.
  3. Click on the TB Requirement and complete the TB Risk Screening Questionnaire, and download the UCSD TB Testing Form if this is indicated.
  4. If testing is indicated, take the form to a Primary Care Provider to order testing and complete the form.
  5. Return to MyStudentChart > Menu > Immunizations and Screening > TB Requirement and upload your completed form.

Regular Immunization Requirement:

  1. Sign up for MyStudentChart, if needed follow instructions.
  2. On the MyStudentChart Welcome Page go to Menu > Immunizations and Screening.
  3. Click on Immunization Requirements.
  4. Enter the dates of the required vaccines — MMR, Varicella, Tdap & Meningococcal.
  5. Upload a vaccine record. If you do not have a vaccine record and/or if you have gaps in your vaccine record, please take the UCSD Immunization Form to a Primary Care Provider to order the missing vaccines or titers (blood testing for immunity) and fill in the form.

COVID-19 Vaccine Mandate:

The University requires students to either receive or affirmatively decline the most updated COVID-19 vaccination annually. 

  1. Sign up for MyStudentChart, if needed follow instructions. 
  2. On the MyStudentChart Welcome Page go to Menu > COVID-19. 
  3. Enter your vaccine type (only WHO & FDA-Approved vaccines are accepted) and dates. 
  4. Upload proof of your vaccine(s).

School of Medicine/Skaggs School of Pharmacy/Atkinson Physician Assistant Education Program

As a School of Medicine,Skaggs School of Pharmacy student or Atkinson Physician Assistant Education Program, you are required to fulfill additional Immunization and TB requirements.

The Immunization and Tuberculosis testing forms must be taken to your Healthcare Provider, and laboratory proof of titers/Quantiferon testing/X-rays must also be uploaded with your forms.
